I might see a patient in the ER with “ chest pain” and I will run labs, do an EKG and get a chest x-ray. I can suspect it if the patient got the vax, the PR interval on the EKG is over 20 and maybe the D-diner ( indicator of blood clotting) could be up a bit. I will observe them for up to 24 hours but no cardiologist will take the transfer in absence of other significant findings.
In the ER we do not have echocardiograms ( not reimbursed for them) and patients must be scheduled outpatient for nuclear medicine studies ( Thalium scans) or Cardiac MRIs ( gold standard for imaging). Since I cannot access the tests that provide the diagnosis of Myocarditis, the patient goes to the house with a diagnosis of “ Atypical Chest Pain” —- and gets scheduled with a cardiologist for follow up. Many patients might not even show up for that appointment. In summary, “ Atypical Chest Pain” is the label behind which many Myocarditis cases are hiding and so the case numbers are well under reported.”
